Since the upgrade from 4.11.1 to 4.11.2 the small icons are set to fixed size 16 although systemsettings tells me they should be 32. Changing it in systemsettings to any other size does not work, the icons stay at tiny size 16. Reproducible: Always

Janet, which widget style do you use? It seems to work with Oxygen style.
Changing the icon sizes in systemsettings, I get error messages in Konsole. Not sure if this is related, or if they have always been there. I've tested it with QtCurve and Oxygen window style. When I change the window style in systemsettings from QtCurve to Oxygen the small icons grow back to the size they are set to, as do the systemsetings overview icons (Bug 325567). When I log out and back in the icons are tiny again. I change the style from Oxygen to QtCurve - everything is ok. Log out and back in - tiny icons. I've also tested it with different user accounts. No matter if I use QtCurve or Oxygen as window style, after logging back in the icons are tiny.
It is Plastique! As soon as I use the Plastique window style the icons become that tiny too. And every time I re-login the style is obviously set to Plastique (though not shown in systemsettings under style.
Sorry, not Plastique but Cleanlooks. They look similar.

I know have tested it on a system with KDE SC 4.10.4 - there the built in styles I mentioned in Bug 325621 already had that behaviour so it is nothing introduced with KDE SC 4.11. And it is not only the small icons it is also the icon size of systemsettings main window which normally uses the dialog icon size (but it does not afflict the module sidebars in systemsettings which also use the dialog icon size).
