SUMMARY


STEPS TO REPRODUCE
System Settings > Mouse > Configure Extra Buttons
Click Add Binding
Press the mouse button for which you want to add a key binding. 

OBSERVED RESULT
No mouse buttons have an effect, except back button takes back to System Settings > Mouse 

EXPECTED RESULT
Should be trivial to configure mouse back and mouse forward buttons (and scroll wheel).

And please consider accepting them as "modifiers" !  This is very important in some use cases, such as "moving a window" while holding an extra button. Most other WM can do it (openbox, wayfire come to mind), but kwin has never been able to do so.

In the meantime I found I can get around this by using https://github.com/sezanzeb/input-remapper

Mine is identified as "SOAI Gaming Mouse" (dmesg) or "Xenta Gaming Mouse" (lsusb).
But I dont think it matters, it provides standard numbered extra keys. It was a breeze to configure with other WM.

FWIW this "Configure Extra Buttons" feature has never worked for me, either.  In the past I've tried it with the Kensington Expert and Slimblade trackballs.  I currently have a Ploopy Adept.

I get to "Enter the new key combination for Extra Button 1:" and it catches keypresses but not mouse clicks, e.g. can't set "Ctrl + left click".
