Hmm, cannot reproduce.
For me it does that if i don't have the backend installed. Installing sonnet, hunspell and hunspell-en_us fix it (I still think the behaviour without backend should be different, should have a warning or something). But from the video, it seem like you have the backend installed. Maybe a reinstall a backend or checking you have all three package (or equivalent on your distro) could fix it(?).
It's from neon-user-20240228-1346.iso live image
Can confirm on Arch, newly installed, with `hunspell` package but no `hunspell-en_gb` or similar dictionary files. Leaving the Spell Check area of System Settings prompts me to save the changes like faveoled's attachment video shows. Installing dictionary files fixes this issue for me (without any other changes). Uninstalling the dictionary files brings back the issue. Saving from the prompt does nothing, next time you leave you're prompted again. The second time I was prompted, the prompt looked weird with a fourth button (see attached image). I didn't click it and it didn't show again.

I can reproduce the issue without the dictionary installed. Installing dictionary resolves the issue.
