Konsole doesn't follow system settings for natural scrolling with mouse wheel, it it seam to be ignored.
It does work using touchpad.

Reproducible: Always

Steps to Reproduce:
1. populate text like using dmesg
2. try to scroll with the mouse wheel

Correct, this would be possible to get the settings from kinputrc or kcminputrc.  However, I don't see a system-wide signal that could be connected to.

However, the entire source code would have be search to handle the change.  Perhaps not too hard but time consuming to find, fix and test.

Doing a search shows that 1) libinput has to be removed and 2) have to restart Konsole (and some other KDE) apps after changing the scrolling.
