I made a stuff up that other people might make as well.
When setting up Kmail to have 2 identities and 2 outgoing network accounts, I did not set the identity->Advanced-> Special transport settings.
It might be more intuitive if the "Identities" icon on the left was below the "Network" icon.  This way the network settings are more likely to be filled in before the identity settings.
Also the "Special transport" title in the "Advanced" tab of the "Identities" section might be renamed to "Outgoing Account" or "Network sending account" or even "Email server".
Is the check box next to "Special transport" needed?
The drop down menu box could be greyed out if there was only one network sending account.
Consider putting the "Special transport" option in the "general" tab rather than the "Advanced" tab.

I agree with observation the settings dialog is not intuitive. Besides, the even doc does not help
Special transport: "Select or enter an alternative SMTP server to be used when sending messages using this identity."

I still don't know alternative TO WHAT is this special transport. In other words, when I leave it empty what happens?

It takes the transport of the default account.
For example, you have two email accounts: yourname@foo.com and your.name@bar.com, with the first as default account. If you try to send an email with the second account and you haven't set the special transport option, you will have an error, because it tries to use your.name@bar.com as login name for the SMPT server of yourname@foo.com.
It's quite a strange thing...

Thank you for your feature request. Kmail1 is currently unmaintained so we are closing all wishes. Please feel free to reopen a feature request for Kmail2 if it has not already been implemented.

In KDE SC 4.8, the "Identities" item is still above the "Accounts" item in the KMail configuration dialog.

P.S.   In my opinion the order of these items is logical and should not be changes.
