save/open file dialog: disable incorrect places

Do:
1) in shell - mkdir X
2) in kate, open file
3) in konq. d&d X dir to open file dialog places pane
4) cancel in kate
5) in shell -- rmdir X
6) in kate, open file

Look at X entry, looks pretty healthy, despite the directory it refers to does not exist. Please disable such entries, so LMB won't work (and they would be grayed out).
