STEPS TO REPRODUCE
1. Went to "Window Management > Desktop Effects".
2. Enabled "Hide Cursor".

OBSERVED RESULT
I had to reboot for it to work.

EXPECTED RESULT
It should work right away, or have a message telling I have to reboot when enabling it for the first time.

Worked straight away for me.
Can you double check and reopen.

Can you also paste your .config/kwinrc after enabling when it's not applied.

Issue on Plasma 6.4.5

I've deactivate the Hide Cursor that was set to 5 seconds, then activated it set to 1 second and it's not working right away.
The option to hide while typing is working fine though.

I've tried to access .config/kwinrc with Dolphin and nothing showed up.

I've rebooted now, and the cursor is disappearing after 1 second.

I've now increased to 5 seconds and it didn't require a reboot. I don't know if it was because it was an increase, or if it was because it was set before.
