SUMMARY
Sometimes when trying to click a panel item (like the application launcher or Wi-Fi settings) it will not recognize a click unless another action is taken in the panel then it will recognize, so a scenario will be as follows:

I try to open the application launcher by clicking it, but it doesn't open.
Then I press the meta key to open it instead and it does open.
Then clicking the icon now suddenly works for sometime then it breaks again.

Doesn't seem to be the same bug since mine happens only sometimes and very random and is fixed just by clicking on any other widget, and I actually don't think it ever happened yet after updating to 6.4.2, but I'll update you if it happens again.

And my displays are as follows:
[3840x2160@60Hz] [2560x1440@144Hz (Primary)] [1920x1080@60Hz]
All of which are set to 100% scalling.

It just happened right now, not sure what triggered it but I was setting up some widgets in my desktop and it happened when I tried clicking the application launcher, but at least 4 or 5 days had passed before it happend again, and it used to happen on an almost daily basis (multiple times a day) before updating, so maybe 6.4.2 had something on making it happen less?

Thanks for the update. Are you able to reproduce this with a brand new user? It's possible there's a cached value or old config value that's causing an issue.

Currently the application launcher is fine and the issue rarely happens in my main account (didn't happen with the new account but i'd imagine that it would happen once or twice if I tested it for a long period of time), but there is a very similar issue with the system tray (could be even the same issue) where clicking a tray entry (like the network icon) would not open for multiple clicks then it would open, or sometimes it wouldn't open until you click on another entry. I managed to catch it in a recording where clicking the audio icon did not open until I clicked it a third time. And this issue also happened in the new account 

I also recorded another issue in the second half of the video which is really strange and happens on multiple accusations, the most reliable one is to just move the cursor while holding left click in the entry, where the icon would go to an inflated and deflated state rapidly.

The bug with the application launcher where it won't open no matter how many times you click it only until you click on another thing then it works again just happened on a brand new installation that shares no config files with the old one.

But still, it happens much less often than how it was before 6.4.2

Thanks for the new information, and the screen recording. That's very helpful, it shows exactly what's happening.
I can't reproduce this and haven't seen it on git-master or Plasma 6.4.3. I'll leave this open so other people can try to reproduce.

Indeed, a very helpful screen recording, especially with the annotations.

I can see the clicks are registered by both the system and the widgets, so it's not a hardware issue or a logic bug in Plasma. It feels like a graphical issue of the popup not appearing on screen. If so, it could be a similar issue to Bug 480800, which was ultimately traced to Qt.
