That sounds bizarre. Could you attach a phone video that shows it happening?
Created attachment 175677 [details] photo of the icons I'm sorry I thought I had attached the photo already. I don't have a video as of now but I'll try to record it next time it happens. It only happens every few days and i don't know what exactly prompts it.

My colleagues say this is likely https://bugreports.qt.io/browse/QTBUG-123799. Still, if it is, we should figure out why failing to unlock triggers software rendering (and hence the bug) for you. To confirm, everything is fine until you fail to unlock, and then it happens, right?
It hasn't happened for a few days now but if it does it's usually after my laptop has been asleep for a bit. And then it only happens once I enter the wrong password. But if I hit escape and then open the password prompt again the icons are back to where they are supposed to be.
