Is the app that spawned the dialog a Flatpak app? Can you reproduce the issue with a non-Fflatpak app that opens the GTK file dialog?

It isn't a flatpak app, but a native RPM for OpenSUSE. Also this behaviour can be seen in system settings when you preview a GTK theme
