The video pretty much shows what's happening and how to reproduce.
The most annoying part is the unexpected opening in the middle of the screen instead of near the clicked position.

FWIW can't reproduce on plasma 6.1 git build, wayland, arch linux.

Is this on wayland or X11?

I also noticed that there seems to be some custom themes used. Does this also happen with vanilla plasma without any 3rd party stuff?

This happens on wayland (did not test X11) but does not always manifest.
I'm using breeze with some color adjustments.
I've seen this on at least 2 separate boots in the last few days, I cannot reproduce it immediately after a fresh reboot however.

Ah, I'm now able to reproduce this reliably on my setup.
I'm using an external monitor and tend to 1st lock my session and then 2nd turn that monitor off.
This has the weird effect, that I'm presented with an extra "unlock" button after successfully entering my users password when coming back. This button works and does unlock the session (although it's presence is weird).

After such a weird unlocking I always see the behaviour shown in the video.

Could anyone who can reproduce this share a screenshot of systemsettings - Window management - window behavior - focus?

It seems that this happens when plasmashell tries to make a popup without being focused. However, I can't seem to reproduce this.

Another observation: I find this most reliably to reproduce when right clicking my panel when some window is active as opposed to clicks on the desktop as can be seen in the video.
Right clicking desktop works as intended (most times if not always in my case) when right clicking my panel produces a new centered window for the context menu.
