I've witnessed this bug a few times for different applications. When I minimize a window, leave it alone for a while, and try to find it later it's gone. The window is actually moved outside viewport so I have to manually use the move button in the menu to move it back. I'm using Wayland. The most recent case is a Librewolf window. IIRC it happened to other windows but I'm not sure. I think on Wayland apps can't move their windows, so it must be kwin ?
Can you confirm if you have any kwin scripts enabled Also can you confirm if librewolf is a wayland application. The easiest test is to run xprop and hover over librewolf if the cursor changes to a + it's running via xwayland

In the settings UI none of them is enabled. I never touched them either.
