STEPS TO REPRODUCE
1. open a document (I know t is not necessary)
2. open settings / keyboard shortcuts, set some changes
3. save the document, including reload...

OBSERVED RESULT
the assigned new hot keys don't work anymore, the new settings have been forgotten

EXPECTED RESULT
the opposite

Windows:
macOS:
Linux/KDE Plasma: Linux mint
(available in About System)
KDE Plasma Version:
KDE Frameworks Version:

ADDITIONAL INFORMATION
It seems to function well at this moment...
the changes haven't been lost, it seems...
but I played with closing the document, closing Okular, and opening...

You are probably using an LTS distribution from 2020, which doesn’t include KDE Frameworks version KF 5.78 yet.
This is what was supposed to fix Bug 384700, which seems similar to me.

Can you provide your software versions?

it functions good now
I don't now what happened, but the problem had produced itself twice...

I now have checked whether it is just the last modification that gets lost...
I did one modification, saved and reloaded the document (and Okular), and it didn't get forgotten...

so, at this point I cannot reproduce it anymore...
Sorry for taking your time!!!
