Cannot reproduce. Do you have any local customizations which might help to explain this? Non-default icon themes, App styles, KWin scripts, etc...? I do notice you're using the Fusion style. Is that intention, or a symptom of using qt6ct or not having plasma-integration installed?

qt6ct : Not installed. Plasma Integration : Installed and Updated. I don't have any customizations. Global Theme : Breeze Dark Application Style is Breeze and not Fusion. Using Fusion or any other style does not help either. Plasma Style : Breeze Icons : Breeze Dark Colors : Breeze Dark Tried disabling all KWin scripts the issue still persists, although any of them should not affect the icons in the first place. Notice that this only happens when Icons are set to Breeze Dark. Breeze (light) icons work correctly. The attachment shows Global Theme set to Breeze Light and icons set to Breeze Dark.
What happens if you use the Breeze icon theme, and not Breeze Dark?

Then something is wrong. When using the Breeze icon theme (not Breeze Dark), the icons are supposed to be re-colored by kiconloader. This is set up by plasma-integration; that's why I asked if you have it installed. If you do, then I'm leaning towards a distro setup problem with the qt6 version of plasma-integration at this point. So I'd like to ask you to have your distro to check their plasma-integration packaging. Can you do that? Thanks!
I did check with my distro and indeed they provide a Qt5 version of the package in the kde-unstable repo. I went ahead and built the Qt6 version from here : https://invent.kde.org/plasma/plasma-integration. The problem still persists. What else specifics should I look into?
