STEPS TO REPRODUCE
1. Plug in USB headphones.
2. Unplug headphones.
3. Plug them in again.

OBSERVED RESULT
Plugging in USB headphones creates a new, matching USB output device in the applet. When the headphones are unplugged, the output device remains and a new one is created when the headphones are plugged in again - filling the menu as shown in the attached image. This happens in the settings menu as well, but appears to reset when closed while the tray applet does not.

EXPECTED RESULT
The output device in the tray should either disappear when unplugged.

Switching between two different sessions of different users seems to be affected too. On every switch + switch back a new set of duplicate devices is added. Volume control only works after switching to the newly added device, see screenshot where the audio volume meter of the old duplicate is inactive. Radio buttons are buggy too and select two or more devices.

It does appear to be the same issue, yes. I did try finding duplicates before submitting but I never saw that one.
