Tried a Wayland session under Plasma 5.27.5. My second mouse doesn’t respect the natural scrolling option. It’s just applied to my first. Works under X.org.
Feel free to move this to the correct product/component.

Settings are mouse-specific. Have you tried toggling the natural scrolling for your other mouse as well in the Mouse Configuration page of System Settings?
