SUMMARY
***
Hello. I was playing around with "move to screen" option today and it is actually quite confusing how the option is shown. So I have two requests:
1. Show screen model names instead of "HDMI-A-0" or "Displayport-A-2" like system settings does
2. List screens in order from left to right. Left most being screen 1
***

How to reproduce:

Connect more than one monitor, right click on a window and select "move to screen"

OBSERVED RESULT

Screen liist shows up like this:

Screen 1: (Displayport-A-1)
Screen 2: (HDMI-A-0)
Screen 3: (Displayport-A-2)

Problems:
1. I don't remember what connections my monitors are using
2. They aren't in order from left to right | correct would be (Hdmi-a-1,DP-a-1,DP-a-2)

EXPECTED RESULT

List shows up like this:

Screen 1: (Vizio M220VA)
Screen 2: (Dell U2713HM)
Screen 3: (Acer R240HY)

Improvements: Easier to read, don't have to remember how monitors are connected.

KDE Plasma Version: 5.24.5
Windowing system: Xorg
