This should be fixed in Plasma 5.25 already. I'm pretty sure, at least. Feel free to re-open the bug report if you upgrade to 5.25 and you find that it's not fixed yet.
I have been experiencing this bug on Arch Linux as well. I will comment again when 5.25 beta hits kde-unstable.
this is still occurring on 5.24.90 on arch Linux. There is a phantom icon - see attached video

Plasma 5.25.2, Arch linux and Arco linux, still happening.

I am still experiencing this bug on plasma 5.25.2 on fedora.
One quick fix if you need it is to right click on the desktop icons>sort, and sort the icons in anyway and it will refresh and remove the zombie icons.

Created attachment 152721 [details] Plasma Desktop - phanton duplicates I got it on X11 as well. Little pre-story: my btrfs filesystem has died recently, so I had to copy-clear-reformat-restore everything from and back to it, so after a day and a half it's finally working again. But when I connected second monitor (which is configured to have files and folders on it, unlike primary one), I found there two phantom icons. They have a property of stacking after the last "real" item on a grid, i.e. if you move some item to some bottom-right position — two phantoms would jump to stay on the right of it.
This should be fixed in Plasma 5.27 (aka current git master) as of various ports and fixes.
(In reply to Nate Graham from comment #18) > This should be fixed in Plasma 5.27 (aka current git master) as of various > ports and fixes. Looking forward to February 2023 then. Recently it seems like it is creating more phantoms, could be subjective. I'm on 5.26.4.
