STEPS TO REPRODUCE
1. For Mouse settings, check Invert scroll direction
2. Suspend system
3. Wake up system
4. Scroll with the mouse wheel

OBSERVED RESULT
Scrolling is not inverted.

EXPECTED RESULT
Scrolling direction is inverted.

Note: unchecking "Invert scroll direction", applying, then checking, and applying again, temporarily fixes the issue only until the system is suspended again.

This is X. I am unable to test Wayland anytime soon.

I'm having the same issue on X11. I've tried using Wayland but I could not reproduce the issue there.

It does not seem to be suspend-related. If I turn off and on the bluetooth on my mouse, I get the same result: scroll direction is reverted back. Also when I switch my mouse from bluetooth to usb-dongle the same happens.

I think you are correct, and I was mistaken. I have a USB wireless mouse, not Bluetooth, and I can reproduce the scrolling problem by unplugging and plugging back in the USB dongle.

I will change the bug title accordingly.

A bug report about this has made it to the 15-minute bug initiative: https://bugs.kde.org/show_bug.cgi?id=435113
So hopefully this will be addressed soon!
