Because KWin installs WindowActionInputFilter in wayland session. But the filter ignores the option.
The expected behavior should be set in Window Actions.

I just tried to change the settings and it still worked. Choose "Activate"/"Activate and pass click" in Left click.
"Window activation policy: Click to focus. Focus stealing prevention: None. Raising windows: Click raises active window - Checked. Inactive Inner Window Actions: Left click: Activate and pass click." With these options, when I try to open a second pdf document, okular's window is not raised and document is opening in the background with a new tab. Same is true also when I try to open a link from thunderbird in a web browser. I can provide anything you want about my system, I do not have any guesses about why this option is not working.

That's a different bug then. I can also reproduce it.
Hım, so there has to be a some configuration then raising windows with clicking works. Could you please share the options that you use? I post the bug tomorrow.
