Severity: normal

The undo menu item should be disabled when the user cannot undo any more 
actions.  If a user presses Undo repeatedly until all his actions are 
undone then the button should be disabled.

The redo menu item should be disabled if a user has redone all of his 
actions.

For instance a newly opened document should have both undo and redo 
disabled because there are no actions to undo or redo.
