STEPS TO REPRODUCE
1. add Folder View widget to your desktop
2. right click on Folder View widget, click on "Configure Folder View..."
3. select "Places panel item" radio button, select "Recent files" in the dropdown, click on "Ok" button
4. right click on Folder View widget, select "Remove Folder View"

OBSERVED RESULT
As we can see in the attached screenshot, Plasma shows "undo" notification with "unknown" icon.

EXPECTED RESULT
Plasma notification should have a more suitable icon

The icon is set by the folder view plasmoid, re-assigning

The bug persists on Plasma 6.5.

So it does; I wasn't testing correctly, as this only affects cases where it's showing a location with a non-default icon. Sorry about that.

Digging into the code, I'm not sure how we could prevent this. The notification is just showing the widget's icon, and the widget's icon is being set to the folder's custom icon.

We would have to tell the widget to always consider its standard icon to be something fixed, and then make the CompactRepresentation show an icon that's *not* its standard icon.

Which is possible. But I'm afraid that would just introduce inconsistency elsewhere that people would complain about.

So I doubt there's anything satisfying we can do here, sorry.
