Reproducible on both Arch Linux (Plasma 5.18 beta) and Neon unstable edition.

STEPS TO REPRODUCE
1. open system settings > account details > user manager
2. click on "+" button on bottom
3. in the first text field, type the desired user name
4. type nothing in "Real Name" and "Email address" fields
5. set the account's password
6. leave both checkboxes unchecked
7. click on "Apply" button

OBSERVED RESULT
user account creation fails silently.
User Manager creates the user account as expected if I type something
in "Real Name" field.

EXPECTED RESULT
user manager can create a user account without a "Real Name".
