SUMMARY
When an application is terminated, the corresponding icon is not deleted from the panel, so subsequent links between icon and application are out of sync. In the attached screen grab, the mouse pointer is on Krusader, but the application highlighted is Xterm. At the time of the screen grab, Kinfocenter was also terminated

Also, panel does not expand to show other applications. At the time of the grab, Krusader had been relaunched, but does not appear.

Can only offer the following additional info, which may or may not be relevant:
1: Video card is GeForce GT 620
2: Graphics effects are reset from time to time
3: Clock display freezes as part of the symptoms 
4: Restart by killall  plasmashell followed by kstart5 plasmashell 
(discovered by trying to find cause of clock freeze problem) is not satisfactory.

Ok, so the real bug is the entire panel has stopped updating?

Have the glow on mouse hover effects stopped too?

As I tried to show in the screengrab, what appears in the mouse hover is not related to the application icon

I can reproduce this  issue. Resolving the ticket.
Please feel free to reopen it if still can reproduce in a newer version(Plasma 5.23.5)

RESOLVED WAITINGFORINFO is a nonsensical status; don't use that. It should be either NEEDSINFO WAITINGFORINFO, or else RESOLVED WORKSFORME.

However...

Yeah, so this is a duplicate of the old NVIDIA panel freeze bug.
