These are two different issues, and Bugzilla tickets should only be used to track a single issue. Let's use this bug to track the OSD issue, and please file a new bug for the duplicated shortcuts issues. Does the issue persist if you use the Breeze look-and-feel or the combination of Breeze for the Plasma theme and icon theme?
You can file it by yourself or don't fix at all. If I stumble upon it again in several years I will file it.
No, I didn't try Breeze look-and-feel or the combination of Breeze for the Plasma theme and icon theme. I have Oxygen Look and Feel and Oxygen icon theme. Please, test your software properly.

I would recommend reading https://community.kde.org/Get_Involved/Issue_Reporting to ensure that your interactions here in Bugzilla are smooth and optimized for getting problems resolved. And please do file a second bug for the duplicate shortcut issue. The reason why each problem needs its own bug report is so we can track them separately in the (likely) event that each problem requires a different code change to fix. Thanks!
duplicated shortcut issue was already reported as bug 408456

It was changed to a smaller, less intrusive style in Plasma 5.20. :)
