This is the `system-software-upgrade` icon, which lives at /usr/share/icons/breeze/apps/48/. It should change to symbolic line-art style for its 22px and smaller sizes.
Still observable on KDE Neon 18.04 -> 20.04 upgrade. In general, it feels like it should be in the same place as other updates are shown and controlled - Discover. But I guess distro upgrade is a different kind of thing technically so it may be tricky to implement
Again, still observable on KDE Neon 20.04 -> 22.04 upgrade :)

Yep, because nobody has fixed it yet. You don't need to keep re-confirming that it's still an issue. Looks fairly easy to fix though.
