If you set the inverse mouse wheel scroll, it works fine until the next reboot. After rebooting, the scrolling is not reversed even if this setting is stored correctly in .config/kcminputrc file and showed correctly in systemsettings. Unsetting and setting back the option solves the problem till next reboot.

This is the mouse settings page. "Inverti la direzione di scorrimento" means "Reverse the scrolling direction". It is set but on startup mouse scrolling is not reversed. I need to unset and set this chek, then apply options to make it work

Ah OK, you're using the old evdev mouse settings page, not the new Libinput one. So this is a duplicate of Bug 395401. You can follow progress there.
