I'm unable to close / lock my Vaults. They remain open/accessible via Dolphin, even after rebooting the PC.

In short, they're stuck at functioning as a normal directory.

In the dropdown, I have the Mount icon on the right side. Clicking it yields a prompt for Password (even though I can access the Vault dir via Dolphin).
Mounting the vault via the Icon doesn't really do anything. No Dolphin window pops up, and there's no indication in the dropdown that the Vault is actually mounted (i.e. no Unmount icon).

Clicking the Vault Name > Open with File Manager prompts password, upon entering it I'm given a new Dolphin window inside the Vault.

You need to upgrade to Plasma 5.13 and the newest cryfs version (at least 0.9.9).

Then recreate the vault.

If this problem persists in 5.13, please reopen this bug report.
