The unreadable headers have already been fixed in Plasma 5.12.3 (see Bug 384638). This looks like more of a problem with your Look & Feel package; the text definitely shouldn't be black in Breeze dark... Does the problem persist after restarting System Settings?

does this still happen on master (and after deleting the icon cache) can't reproduce here
