If I change the volume of some sound playback device using plasma's volume icon tray, then whenever I change the volume of an individual application, the volume for the device is also changed.

Combined with bug 390394, this makes for a reeeally annoying experience when using high-volume headphones.

You need to disable flat volumes in pulseaudio.
