The rule.

When I try to create a new file, or a new folder (it can happen in other applications I am sure), the filename/foldername dialog that appears in the task manager is minimized EVERY TIME. It also is orange. At first I thought that it didn't appear but it was just minimized.

I managed to fix the problem by adding the window rule you can see in the description.

The Task Manager doesn't minimize any windows. We need to assign this ticket somewhere else.

Where do you open this dialog from?

It happened to me on the desktop before I made that rule. It was very annoying. Not that big of a problem if it can't be reproduced, though my system is quite stock.

Maybe you changed focus stealing prevention rules? Could you please tell in what apps dialog windows appear minimized?

Focus stealing is at low. I don't thing I ever did anything to it. Also this appeared when trying to create new files in plasmashell. Right now I am not sure if my kwin rule is keeping it from happening or something else got fixed and also fixed this. (I tried to deactivate the script, it doesn't seem to be a problem anymore, I also restarted plasma just to be safe and it seems ok. I will not say is gone, though is not the first time when a bug kinda gets fixed by some new updates)

I have exactly the same problem. 
@RaitaroH - thanks for temporary solution. 
When I enable the rule and disable it again the problem is still there, so it's reproductible.

Forgot to add: currently on Kubuntu 18.10 with Plasma 5.13.5, but had this problem also before.

Cannot reproduce without any window rules set up. If you have some and they are related, I strongly suspect that they may be the culprit. If this is still happening for you in Plasma 5.19, can you try removing the window rules and seeing if the problem goes away?

This problem has been resolved it seems. And no, it wasn't a rule issue. I will put in the "hmmm, kde ¯\_(ツ)_/¯" box.
