There are duplicate entries under the Recent Documents category when a file is opened more than once. E.g. if I open filename.txt n times, even if it is in the same application, it shows up n times under Recent Documents (where n is less than or equal to the max. number of allowable entries).

This is also a problem for Application Launcher, but only when searching. If you navigate to the History tab, there are no duplicates.
