KMail is not always showing the security warning to display HTML messages even the mail is in the HTML format.

The side bar considers the mail as "raw text" even if it is HTML. 

Workaround: select "Prefer HTML over raw text" in Security settings.

Even though I have "Prefer HTML over raw text" selected, it still displays raw text the first time an email is displayed.

Even if I set the "message default format" to prefer HTML or set it in security settings, it will go back to displaying text or not displaying images when I go back to that folder later.

I can confirm this. Most of the mails with both plain text and html are almost always displayed as text only with no HTML option.

From using KMail in KDE since 2003/4, it worked superbly until Akonadi was introduced into the mix. Akonadi from my experience corrupts not only KMail but it screws with Kontact PIM programs and also is possible responsible for other of my programs failing to execute scripts, or however it works, that screws with my computer/s both (x2) laptops and my 2 previous desktop computers and continues on new main desktop computer. I have been opting to change back to OS2 Warp, Apple or 'Heaven Forbid' M/S BSOD or I will be finding a means to operate without Akonadi altogether. I have retired but I have IP commitments that cause me to be extremely busy and time poor to bug detect/report and the hassles are getting too much to put up with for much longer. At present I just need a system that works as it should.
My computers are all high end AMD based systems.

this bug has been fixed. but I don't think the fixed version will be released until 2018.  Laurent do you know which version will have the fix?

I also cannot reproduce the bug with the attached test case anymore. Nowadays it seems to work correctly with KMail 5.9.2 (Qt 5.11.2 / KF 5.51) as well as KMail 5.14.40 (Qt 5.14.1 / KF 5.69).

Therefore closing the bug.
