This appears to be intermittent and occurs sometime during a session. 
This is version 16.04, a clean install a few days ago.
This appears to be a KDE problem.

Reproducible: Always

Since my original post and on a fresh install on two occasions I spotted when the effect occurred. The first was on trying to expand a window with its title bar rectangle. At which point all the title bars on opened windows disappeared.
