A rounded empty square can be seen on right side of the text bar. The square is a placeholder for the "Jump to next match" button. The square and the tooltip appear when the mouse pointer is on it.

With
Kubuntu 16.04 Kate 15.13.3 Frameworks 5.23.0 Qt 5.5.1
or Neon devedition-gitstable Kate 16.08.0 Frameworks 5.25, Qt 5.7.0
or Latest Frameworks, Plasma + Kate build from sources
all icons visible
You have to provide more info, but for me this does not look like a Kate issue

Looks like missing icons, for me no Kate issue, e.g. breeze icons ship the needed ones.
