If one attempts to open a document from Kmenu history, there is an error: Could not find any application or handler for </path/to/file>.  However, if one right clicks said document from KMenu history and selects an application from the 'open with:' option list, the document opens without an error.


Reproducible: Always

Steps to Reproduce:
1. Open a document, which could be a text file, pdf document, image file, etc.
2. Close the file, then go to KMenu history or recently opened files and click said file.


Actual Results:  
Could not find any application or handler for </path/to/file>

Expected Results:  
The document should open using its default application.

This bug was confirmed on openSUSE Tumbleweed (20160526),  Kubuntu 16.10, and by another individual running Linux from scratch; all the systems are running Plasma 5.6.4.
