This happen sometimes, but has happen more than one for me.
After I reboot I will notice that my dual monitor setup has switch to single monitor mode( both monitor show the same thing, screen size is reduced)
When I open the monitor setting I only see 1 monitor, this is a bug!!!
I didn't realize the one of the monitor was overlay on top of the 2nd monitor in the settings dialog box, only when I decided to move one did I see the other and was able to get dual mode working again. No one in their right mind would ever think to do this!
This should never be allowed to happen, if someone wants this effect, there should be some other setting visible on the same page to say mirror monitors or something like this.

Exactly the same problem. With 2 screens, at start, the configuration reseted switching from "extended" mode to "monitors show the same thing" mode. 
Not happen each time, but often.

When I switch back in kscreen from "monitors show the same thing" to "extended mode" the configuration of the second screen is reestablished (panel, plasmoids...).

Using Plasma 5.5 on openSUSE Leap 42.1.

I can confirm this (kde plasma 5.5 on kubuntu 16.04).

Hi!
Same problem.
The dual screen configuration is not preserved. Only disabling kscreen permits (System →  Start and Stop Background Services → Service KScreen 2).

My config:
    KDE Plasma: 5.6.3
    KDE Frameworks: 5.21.0
    Qt: 5.6.0
    Kernel: 4.1.20-11-default
    Nvidia drivers: 361.42

Screens become one, or laptop screen gets disabled.

Graphics: nVidia 387.22

The same situation. For 2 years already.
Manjaro
Plasma: 5.11.4
Linux 4.14.5
nVidia-bumblebee, Intel used for Plasma.
