Headphone don't work after bootup if they are already plugged in. I have to pull them out and then plug it back in.

Reproducible: Always

Steps to Reproduce:
1. Plug headphone in
2. Turn off pc, turn on
3. Listen to some video clip and can't hear the audio


Actual Results:  
No audio

Expected Results:  
Audio

Headphones need to be pulled out and then plugged back in for the audio to work again.

Hello the same things with the headphone happen when I put my computer to sleep overnight and then wake it up. The headphone stop working and need to be pulled out and then plugged back in to work.

Problem still exists with headphone jacks, when I unplug the headphone and plug it back in I no longer see an option for headphone under "Audio - System Settings" dialog or Pulse Audio dialog.
