On most screens the fly-over help does nothing (other than tell you what "Apply" does)

Expectation: Help button should bring up help!

Which application is this report for? Do you mean the ToolTips (just hovering), or the What's This mode (clicking "?" icon in the window title bar)? What element is missing help, and which information should be added there?

I mean the "What's This mode" - and it seems to do nothing in most places: most System Settings (other than the Apply button, if there is one), most dialogs, etc.

Should I put in a bug report for each? That seems like a bit much, even for me. :)

Yes, there are many elements without additional help. Note that adding texts to any and all elements would also exponentially increase the work of our translation teams, so we try to be conservative. I think there was a report suggesting to hide the What's This icon from the windows by default, but I cannot find it right now.

I would suggest to report it only for applications where you really needed more information, because the label or tool tip, common sense and intuition alone was not sufficient to understand how the element or button is to be used, and the documentation or Help file is missing or did not clarify either.

I am closing this one. Please report specifics individually for the applications.

In the future, please report against the product/application where you notice the issue, not everything against product "kde". Then we only have to move them if the product is not correct (e.g. for dolphin vs. kio issues).

I think the hiding of the "What's This" icon would be a great solution… no need to add "help" text if it is not needed (and would be silly and a big time waster for not just developers but, more importantly, users).
