Comment 3 Yuri Chornoivan 2014-03-15 07:40:41 UTC
It seems that the document is corrupted.

1. Okular does show the content. It can be reached near the end of the scrolling area on the right border of the Okular window. The rendering of the first page is broken.
2. QPDFView shows the first page as thin vertical line (the same poppler version is used).
3. Foxit (under wine) does not show the first (phantom) page at all.
4. ToC is broken for all applications.
5. pdf-tools ( http://www.pdf-tools.com/pdf/validate-pdfa-online.aspx ) validator shows the following log for this file:

Validating file "ddd.pdf" for conformance level pdfa-1b
The separator before an 'endobj' must be an EOL. (279)
The key Metadata is required but missing.
The separator before 'endstream' must be an EOL. (280)
A device-specific color space (DeviceRGB) without an appropriate output intent is used.
The dictionary must not contain the key 'LZWDecode'. (13)
A device-specific color space (DeviceCMYK) without an appropriate output intent is used.
A device-specific color space (DeviceGray) without an appropriate output intent is used.
A device-specific color space (Annotation C or IC) without an appropriate output intent is used. (111)
The key F is required but missing. (111)
The document does not conform to the requested standard.
The file format (header, trailer, objects, xref, streams) is corrupted.
The document contains device-specific color spaces.
The document contains hidden, invisible, non-viewable or non-printable annotations.
The document's meta data is either missing or inconsistent or corrupt.
Done.

Okular can display any page here other than the first page. I don't see how this is an okular bug. Are you sure you can not see the rest of the pages?
