I suspect that KMail when filtering using a regular expression uses case insensitive regular expression because a subject filter ^[A-Z]+$ also acts on the subject 'test'.

It would be nice if it was possible have an option for case sensitve matching.

This is a valid wish - but I would not implement it without a good idea of adding no further clutter to the UI.
