I have GTX560Ti with only one HDMI output but KDE settings show two HDMI outputs + two HDMI 5.1 outputs. Only one of them works.

PHONON_PULSEAUDIO_DEBUG=5 kcmshell4 phonon

Not sure that will actually help, but it might. This issue most probably is a bug in PulseAudio since the code in question has worked as expected for some 3 years and not shown any such behavior.

(Probably) A pulseaudio bug. There's nothing indicating a duplication inside phonon or the KCM, so PulseAudio must actually think there's multiple devices with the same name. Please report the bug there.
