When safely removing an USB device, it remains in the panel until it is physically removed from the USB port (which is so far ok).
Some devices (e.g. smartphones, here a HP Pre3 running webOS) support making the device mountable again by triggering an action on screen like "Switch to USB mode".

This causes the following issues in the places panel:

- The label of the device is set to "Unknown" once the device has ben "safely removed", so it could be removed altogether instead as it doesn't provide any valuable information at this point.

- When making the device mountable again, it keeps being shown as "Unknown" until one makes the view redraw (e.g. move the mouse onto the device entry in the places panel or switch the focus to another window)

- Clicking on the re-connected device doesn't show any content in the file/directory view. Content is only shown when clicking on another place first or using F5 to refresh the view.

Do you still have that issue on a more recent KDE/Plasma?
