As described in the forum post https://bbs.archlinux.org/viewtopic.php?pid=1334594#p1334594 , a recent upgrade to GNOME has made many icons disappear under the oxygen-gtk theme.  I'm not clear as to the exact cause, but discussion in that thread seems to indicate an issue with "fallback" icons.

Switching away from oxygen-gtk theme and oxygen icons - say to QTCurve and GNOME icons - makes the problem go away.

Using:
oxygen-gtk2 1.4.0
oxygen-gtk3 1.2.0
oxygen-icons 4.11.2
gnome-icon-theme 3.10.0
gnome-icon-theme-symbolic 3.10.0


Reproducible: Always

Steps to Reproduce:
1. Choose oxygen-gtk theme and oxygen icons
2. Observe many icons missing (see http://www.darose.net/OxygenThemeIconsMissing1.png and http://www.darose.net/OxygenThemeIconsMissing2.png)
3. Choose QTCurve theme and GNOME icons
4. Observe icons returned (see http://www.darose.net/QTCurveThemeIconsPresent1.png and http://www.darose.net/QTCurveThemeIconsPresent2.png)
Actual Results:  
Many icons missing when using oxygen-gtk theme.

Expected Results:  
All icons should be present when using oxygen-gtk theme.

Unfortunately, it's a gtk3 bug, and has been reported as such to them already

Temporary solution is to use gnome as icon theme 
(that's pretty much the only one that works ...)
