KMix should select the primary output channel based on the Phonon's devices priority and wheter a device is available and not via that dialog box in Kmix...
For example the Phonon's device priority list is:
1 - USB Headset
2 - Internal Audio
3 - Simultaneous output to Internal and HDMI
4 - HDMI Output 

The Kmix primary output channel is set to the USB Headset.

Reproducible: Always

Steps to Reproduce:
1. Attach the USB Headset
2. Press Volume + / -. It changes the volume output of the USB Headsets
3. Detach the USB Headset
4. Press Volume + / -. 
Actual Results:  
It changes the volume of Simultaneous output to Internal and HDMI

Expected Results:  
It should change the volume of Internal Audio (becuase it is set so in Phonon).
