That "Minitools" submenu uses icon minitools, but that icon is only available in the monochrome icon theme and absent in the oxygen icon them. So in an default KDE installation, that submenu will have an blank icon. See the attached screenshot.

Shouldn't this be reported vs Oxygen instead of Konqueror especially in light of the fact that Minitools is a Konqueror plugin and not part of Konqueror itself ?

I really don't know. I reported against konqueror because I couldn't find suitable products for konq-plugins and konqueror looked like the most related product. Feel free to reassign :)
