In System Settings > Display and Monitor only my main screen is detected.  xrandr also fails to detect the second screen.  However, nvidia-settings detects both displays and allows me to activate the second screen.  

All windows opened from the second screen open on the primary screen despite making application specific window rules telling kwin to open windows under the mouse, which is on the second display.  Opening the same applications on the primary screen produces the expected result: the application opens under the mouse.

Did this work before? If yes, what did you change?

FWIW: I have no problems with detecting and setting up a second screen, using a Lenovo laptop with an external Dell monitor, Kubuntu 12.10 Quantal beta, KDE 4.9.0. Intel HD Graphics

Yes, it was detected before.  My second monitor disappeared after the upgrade to 4.9.0.

You should try a newer NVIDIA driver (>= 302), which is reported to have xrandr 1.2 support. Hopefully it exports the second detected display to xrandr.

Updated to Nvidia 304.37.  My second screen is now detected in System Settings > Display and Monitor but I still cant get any windows to open on the second display.  All window behavior rules are ignored.

Found that the problem was with Oxygen-transparent style, not with KDE itself.  Ive Closing bug report.
