I cannot reproduced that with these systems:

1) master compiled from sources:
$ okular --version
xxQt: 4.8.0
KDE Development Platform: 4.7.3 (4.7.3)
xxOkularxx: 0.13.80

2) branch 4.7 compiled from sources:
$ okular --version
Qt: 4.7.2
KDE: 4.7.3 (4.7.3)
Okular: 0.13.3

3) Kubuntu 11.04:
$ okular --version
Qt: 4.7.2
KDE: 4.7.2 (4.7.2)
Okular: 0.13.2

Cups 1.4.6 + libpoppler 0.16.4 on all systems

In all three systems I have Copies, Options, Pages, Job, PDF Options tabs in the Print dialog.
Comment 2 Albert Astals Cid 2011-12-04 15:31:07 UTC
That dialog is not controlled by Okular, we just use kdelibs. Assigning to John that is the man behind all that magic.
Comment 3 John Layt 2011-12-06 23:10:05 UTC
I initially suspected it was a problem with Cups 1.5 and Qt, but I have that now on openSUSE and I get the options OK. I'm using Cups 1.5.0 and Qt 4.7.4.

Do you get the extra tabs in the print dialog for other KDE apps like KWrite if you run them at the same time?

Is the printer connected directly to your PC, or via network?

The options are not available if Qt cannot detect that Cups is being used for the printer and falls back to the default lpr setup. Are you absolutely sure that your Cups is working and being used for printing? I _think_ the fact that the "Type" field on the left hand dialog in your screenshot shows "LBP1120@localhost" rather than the Cups name shown on the right indicates that Qt does indeed believe Cups is not available and so is using lpr.

Right part of the attachment was taken at the same time with left in another instance of okular, only with different file opened.

local printer, shared with anonymous access and avahi

Yes, found everything I've printed in the cups' logs

So you have 2 Okular windows open, one with a PDF and one with a a different doc, and the PDF doesn't show the extra tabs, but the other one does? Now that is confusing. What type was the other document you had open in Okular?

@qdiesel.mipt

This bug is quite old. Do you still encounter the described problem with current versions of Okular?
