Version:           2.1.1 (using Devel) 
OS:                Linux

When I want to chose to answer to all I can't use the small arrow on the icon for reply since no sub menu appears.

I then have to right click in the message and chose the option under the menu entry "Reply".

Reproducible: Always

Steps to Reproduce:
Open Kmail and an email
Try to click on the small arrow in the icon "Reply"

Actual Results:  
The mail editor opens

Expected Results:  
A sub menu appears with the entries for "reply to all", "reply to list" etc.

It doesn't appear if you keep the button pressed ?

No, neither on my desktop pc nor on my notebook.

The submenu should appear when the button has been pressed (no matter where) for over ~1 second. If this doesn't work for you my guess would be that there is an issue with your setup. What distro, Qt version, KDE version are you using?
