If I enable my external monitor in systemsettings both monitors, i.e. the notebook's and the external one become black. I can see the mouse pointer but no graphics. If I issue a ALT+F2 and execute kwin --replace the new settings are shown correctly, i.e. the external monitor shows the desktop.

To reproduce, activate an external monitor. This puts the "do you want to keep those settings" dialogue on the external monitor, which IMHO opinion is a bug. Do not do anything and wait until it times out. The screens will then try to revert to some setting and become black. Now you have to issue kwin --replace to fix the issue.

is this still an issue with a later version of KDE SC (e.g. 4.9)? The handling of changing screens inside KWin got changed.
